What this event is
The event provides group education, peer support, and optional therapist-led activities for people who have completed the initial Vivistim protocol. Caregivers may also attend.
The group education and activities are not individualized occupational therapy, physical therapy, medical care, or medical advice. Participating in the group session does not create a therapist-patient or provider-patient relationship with the group leader or Mobia. The group leader does not evaluate, diagnose, or treat you individually during the group session.
Keep working with your own doctors and therapists. Bring questions about your health, recovery, or Vivistim device to them, and continue following their instructions.
These terms cover event participation and the optional device checks described below. Any separately arranged clinical care or device programming is subject to its own arrangements.
Optional device checks and device information
You may choose to have a Vivistim device check at the event. Before the check, Mobia staff will explain it and ask for your separate permission. You may decline the check and still attend and join the event activities. Declining does not affect your device warranty, access to Mobia support, or care from your clinicians.
If you agree, Mobia may read, download, retain, and use information about your device’s function and use for device support and internal review of device performance and use. These terms do not change or override any separate study consent or agreement governing your information.
Taking part is your choice
The event is free and voluntary. Your decision to attend, or not to attend, does not affect your Vivistim device, its warranty, any service or support Mobia provides, or the care you receive from your clinicians.
You may observe without joining physical activities, skip any activity, or leave at any time. You must be 18 or older to register.
Before joining physical activities
Talk with your physician or another treating clinician about whether the activities are appropriate for you. Join physical activities only if your clinician has cleared you to take part.
Tell event staff if you need assistance or an accommodation. If you normally rely on a caregiver for movement or personal assistance, arrange for that support during the event.
Your responsibility during the event
Some activities involve physical movement. You are responsible for:
• monitoring your physical condition;
• knowing your limitations;
• following your clinician’s instructions and the group leader’s safety directions; and
• deciding whether an activity is appropriate for you.
The group leader provides group instruction and may not be able to watch each participant continuously. Ask for help when needed and do only what feels safe.
If you experience pain, muscle strain, dizziness, unusual fatigue, or another concerning symptom, stop the activity, notify the group leader or event staff, and consult your clinician.
Risks you accept
Physical activity carries ordinary risks, including injury, muscle soreness or strain, fatigue, falling, overexertion, and worsening of an existing condition. By joining an activity, you accept those ordinary risks and agree to stop any activity that is not safe for you.
This acknowledgment does not release Mobia, the event host, or the group leader from liability for negligence or other wrongdoing.
Emergencies
If you have a medical emergency, stop the activity, alert event staff, and call 911 or ask someone to call for you.
If you are injured or have a problem with your device during the event, tell the group leader or event staff and contact your own care team.
Photographs and use of your likeness
The session will not be audio- or video-recorded.
Mobia may take photographs of participants who separately agree to the optional photo permission during registration. Agreeing is not required to attend or participate.
If you agree, Mobia may use your photograph and likeness for education, program review and improvement, marketing, communications, and publications, including its website, patient education materials, and presentations. This permission is worldwide, ongoing, and royalty-free, and you will not be paid. Mobia will not publish your name without separate permission.
You may withdraw permission for future use by contacting us. Mobia cannot recall material it has already published.
Please confirm your photo choice with event staff at check-in. If you decline, staff will arrange for you to remain outside photographs without limiting your participation.
Please do not photograph, record, or share another participant’s image or statements without permission. Do not repeat outside the group anything another participant shares about their health. Share only what you are comfortable discussing with others; Mobia cannot guarantee that other participants will keep it private.
Your information
To organize the event, Mobia collects:
• your registration details, including your name, email address, phone number, city, and ZIP code;
• the participant category you select, such as stroke survivor or caregiver;
• your attendance; and
• any event feedback you choose to provide.
If you provide an emergency contact’s name and phone number, Mobia uses those details only to seek assistance on your behalf if an urgent concern arises.
Mobia uses this information to organize the event, send event reminders and follow-up information, and review and improve the program. Mobia shares information only with its staff, the group leader, and event support personnel who need it for these purposes and are required to protect it.
Mobia does not sell this information. Mobia keeps it only as long as needed for these purposes and to meet its recordkeeping obligations.
You may ask what event or device-check information Mobia holds about you, request correction or deletion, or withdraw from the event by contacting us. Withdrawing does not undo prior uses, and Mobia may retain information it is required to keep.
Please do not include medical details or information about other people in registration or feedback fields.
Changes to these terms
Mobia may update these terms. If a change materially affects your participation or the permissions requested for an event you have already registered for, Mobia will notify you and obtain any additional agreement needed before applying that change.
